Players interested in playing in the Susquehanna League who will be 18 before December 31st, 2009, should fill out this form. Your information will then be distributed to teams looking for players and you will be contacted for placement on a team in the league shortly thereafter.

The winning team of each game should report the score and other info via this form for updated standings.

Team Information Form
Managers of all teams are asked to fill out this form to provide the league office with their pertinent information. After initially filling this out, if anything changes at anytime, managers are asked to re-submit this form with the updated information.

Umpire Evaluation Form
Losing team managers can fill out this form to evaluate umpires at any time (winning team managers have the opportunity to rate the umpires in the 'Report Score' form). The information you provide will help us in putting the best possible product on the field.

Reschedule Postponed Game
This form is for use by the league Commissioner or Secretary ONLY to reschedule postponed games which will let all parties know of those changes.
