Hot Stove Release Form
Fill-in PDF form for boys class H, G, F, E and EE.

OGSO Release Form
Fill-in PDF form for girls class A, B, C and D.

OGSO Charter to Charter Team Release
Fill-in PDF form for a team from one charter released to play in a second charter.

Local to Local Multiple Release
This form should be completed only when all players in a league classification from one local are bing released to another local.

Tournament Refund Form
This form is used to request a refund of pre-paid tournament fees. One submittal per local, please.
