NBBSA's charter, the Babe Ruth league, is requiring us to complete background checks annually for all managers and coaching beginning with the 2014 season.  All background checks will be completed by First Advantage.  See attached link if you have any questions.  http://www.baberuthleague.org/background-checks.aspx.

All information obtained is strictly confidential.  NBBSA has a law enforcement officer on our board and will be given all forms for safe keeping.  He will also be involved with the processing of the background checks.  Once the background checks are complete, the forms will be destroyed.

This form must be completed by the Manager and forwarded to the Player Agent within 72 hours.  It will be kept on file in case a claim is made to Babe Ruth for reimbursement of medical expenses. Claims should be made within 60 days of accident. Primary coverage is personal policies and Babe Ruth is secondary.

Volunteer coaches who need verification of having attended the Rutgers S.A.F.E.T.Y. Clinic may request a
replacement card by sending a check or money order payable to "Rutgers Y.S.R.C." for $5 with your name,
address*(see below) and daytime phone number to:
