The following items are required for registration:

1. Parent or Legal Guardian

2. Completed Registration Forms (Click form name to download. Many of the fields on the Pop Warner PDF forms can be entered on the computer, saved and printed and brought with you to Registration.  TIP!!  Before entering information, download the forms to you computer hard drive versus filling them out in the browser window.  Please complete ALL applicable fields):

A. 2016 Pop Warner Participant Contract and Parental Consent Form<-DOWNLOAD
B. 2016 Pop Warner Physical Fitness & Medical History Form<-DOWNLOAD

3. Official Birth Certificate (Please bring ORIGINAL and a "copy" with you to provide at Registration). Returning 2015 Sun Devil Participants DO NOT need to bring their birth certificate.  We have a certified copy on file.

4. Copy of 2015 - 2016 full year school report card* (2 copies - Must show all 4 Quarters and Final Grade - (needed by July 12)

5. 2x3 Photo of your Child (Face shot only; no hats or sunglasses permitted)

Please note: For the 2016 Season, the participants will need to use their own family physician for Physicals; the Sarasota Sun Devils will not have a league Physician available for Physicals.

*A "full year" report card will be required before any child will be eligible to participate. Report cards are due no later than 7/12.

All Physicals must be performed and signed in the year 2016.

No physicals dated or performed in 2015 will be accepted.
This includes the date the physical was performed on the child.
Physicals must be done on the Pop Warner physical form.
Physicals on a Sarasota County School form will not be accepted.
NO FAX COPIES will be accepted.
League rules require the form to be the original document.

ALL SIGNATURES MUST BE ORIGINALS.
