Required documents –

Copy of final report cards for the 2008/09 school year

Physical on Pop Warner form

Copy of health insurance cards

New participants must provide a birth certificate.

Pop Warner rules do not allow your child to take the field for practice on August 1, 2009 unless all required documents have been submitted.
