RELEASE BY PARENT OR GUARDIAN: I, the undersigned, as a parent or guardian of the above applicant for entry into West Windsor Wildcat Jr. Football Club, do hereby give my permission and approval for the applicant's participation in all Club activities during the 2017 season. I further agree to assume all risks and hazards incidental to such participation, and do hereby waive, release, and absolve the organizers, sponsors, board members, directors, coaches, and participants from any claim arising out of injury to the applicant, my son/daughter/ward.
MEDICAL: I give my consent for any emergency medical/surgical treatment to be given to my child. This treatment pertains to any life-threatening situations or other medical emergency such as fractures and suturing. I would like the doctor indicated above to be contacted, if possible. In the event my doctor cannot be contacted I give my consent for emergency medical treatment to my child.
COMMITMENT: I promise to accept full responsibility for all equipment issued to my child/ward and to return in the same condition as when issued. I will return the equipment at the time and place specified by the club and in clean condition.
PHOTO RELEASE: I understand that photos will be taken by the team photographers. The photos will be displayed on the Wildcat website, Facebook and available for purchase.

Please continue to check the web site for further information. We will hold a weigh in, equipment handout, photo shoot and the collection of all required paperwork in mid July. Please check the web site for exact dates and times.

First year players are required to have an original copy of their birth certificate. This document will be part of the official player roster book. All players are required to present a medical release from your family physician. All medical releases must be dated 2017 and have the physician’s signature and medical address stamp. You can download a medical form from our "Handout" section of this web site.

All paperwork must be handed in prior to equipment being issued. No exceptions.
