2021 FALL TACKLE RSVP FORM

It has been too long since we hit the field for some tackle football!

Now that Ontario has entered Phase 3 of the COVID-19 reopening plan we can start looking at the possibility of running a fall tackle season but let's be honest- we are a not-for-profit organization and what we charge in registration fees is literally what keeps us running in addition to being a registration organization with Rama Gaming.

That being said we need to know IF YOU WANT TO PLAY?

PLEASE COMPLETE THIS FORM BY JULY 30TH SO WE CAN DETERMINE IF IT WILL BE POSSIBLE TO FIELD TACKLE TEAMS THIS SEASON.
Note: Registration fees are not due at this time, we just need to verify interest

We would like to field teams at the following age levels:
Peewee - Born in 2010, 2011
Bantam- Born in 2008, 2009
Jr. Varsity-2006, 2007
Varsity- Born in 2003, 2004 and 2005

Note - Due to the uncertain situation surrounding high school football in the fall we are try to plan for a team at this age group also.
If individuals withdraw, after registration, due to a high school football programs the MMF will refund all registration fees minus an admin fee.

Practices would begin in August and run 2x a week
Games starting the end of August/Beginning of September on Saturday or Sunday
Registration Fee would be approximately $450
