WAIVER INFORMATION

I/We acknowledge that I/we are to provide an athletic supporter, cup and a heart guard for my child's protection. I/We hereby grant permission for my child to participate in Spotswood Youth Baseball/Softball League (SYBSL) and will not hold any individual, Board Member, certified coach or parent responsible for any injury to my/our child sustained under normal game and practice situations. I/We also agree to and authorize my/our child to participate in all SYBSL activities on and off the field. BIRTH CERTIFICATES will be required for ALL children that register. BACKGROUND CHECKS are required for ALL Board Members, Managers and Coaches. All parents must sign and abide by the league Code of Conduct.

I/We understand that my/our child may be placed in a division either above or below the division that is determined by their age. This may be done at the discretion of the SYBSL Board of Directors and Certified Coaches for the safety of my/our child and the other children in the division. I understand that my/our child will not be placed on a roster or receive a uniform until payment for registration is processed.

Code Of Conduct

I pledge to be responsible for my works and actions while attending, coaching, or officiating a Spotswood Youth Baseball/Softball League event and shall conform my behavior to the following code of conduct:

1. I will not engage in un-sportsman-like conduct with any coach, parent, player, participant, official or any other attendee.
2. I will not encourage my child or any other person to engage in unsportsman-like conduct with any coach, parent, player participant official or any other attendee.
3. I will not engage in any behavior that would endanger the health, safety or well being of any coach, parent, player, participant, official or any other attendee.
4. I will not encourage my child or any other person to engage in any behavior which would endanger the health, safety or well being of any coach, parent, player, participant, official or any other attendee.
5. I will not use drugs or alcohol while at a Spotswood Youth Baseball/Softball League event and will not attend, coach, officiate, or participate in a SYBSL event while under the influence of drugs or alcohol.
6. I will not permit my child, or encourage any other person, to use drugs or alcohol at a Spotswood Youth Baseball/Softball League event and will not permit my child, or encourage any other person, to attend, coach, officiate or participate in a Spotswood Youth Baseball/Softball League event while under the influence of drugs or alcohol.
7. I will not engage in the use of profanity.
8. I will not encourage my child, or any other person, to engage in the use of profanity
9. I will treat any coach, parent, player, participant, official or any other attendee with respect regardless of race, creed, color, national origin, sex, sexual orientation or ability.
10. I will encourage my child to treat any coach, parent, player, participant, official or any other attendee with respect regardless of race, creed, color, national origin, sex, sexual orientation or ability.
11. I will not engage in verbal or physical threats or abuse aimed at any coach, parent, player, participant, official or any other attendee.
12. I will not encourage my child, or any other person, to engage in verbal or physical threats or abuse aimed at any coach, parent, player, participant, official or any attendee.
13. I will not initiate a fight or scuffle with any coach, parent, player, participant, official or any other attendee.
14. I will not encourage my child, or any other person, to initiate a fight or scuffle with any coach, parent, player, participant, official or any other attendee.
15. I will not abuse the fields, field equipment, and facilities or supplies either owned or utilized by Spotswood Youth Baseball/Softball League.
16. I will promptly return any and all equipment belonging to Spotswood Youth Baseball/Softball League within seven days after the conclusion of that season.

I hereby agree that if I fail to conform my conduct to the foregoing while attending, coaching, officiating or participating in a Spotswood Youth Baseball/Softball League event I will be subject to disciplinary action by the Spotswood Youth Baseball/Softball League Board of Directors, including but not limited to the following in any order or combination:
1. Verbal warning issued by the Board of Directors of Spotswood Youth Baseball/Softball League.
2. Written warning issued by the Board of Directors of Spotswood Youth Baseball/Softball League.
3. Season suspension or multiple season suspension issued by the Board of Directors of Spotswood Youth Baseball/Softball League and will result in the loss of the player’s work bond.
