The 2019 Football Application is currently available. The application may be typed on-line, printed, signed, and mailed into the address below. Some portions of these forms requires original initials and signatures. These cannot be typed and must be handwritten. To be accepted, the entire form must be completely filled out and signed.
Tackle Football Registration Fee $400.00 ($425.00 after June 30th)
Flag Football Registration Fee $250.00 ($275.00 after June 30th)
Service Fee: Each Participant is also required to provide a second check for $75.00 for Service Fee. (This will not be accepted through PayPal.) This check will not be cashed. Parents are required to provide minimal volunteer services during the season. If services are not provided, the service deposit is forfeit and check will be cashed. If services are provided, the $75.00 check will be returned after the season

Payment Options
This year we will be accepting registration payments via Pay Pal (except service fee checks). There is a slight increase in fees if you opt for Pay Pal to cover our costs. (Even if you opt to use this service, you must still fully complete the registration form along with the forms below and mail it into the P.O. Box above.) The Pay Pal option can be accessed from the Home page screen to the upper right of the Gladiator Eagle logo. Click the link below "Registration Online" and this will take you to the Pay Pal page.
If you pay by check, please mail your check with your registration material. If you pay using Pay Pal, please print your Pay Pal receipt and attached to your application when you mail in your registration material.
Partial payments for tackle and cheer of $200.00 (check) or $210.00 (Pay Pal) will be accepted with the application so long as the balance is paid prior to June 1, 2019 (the date set for equipment handout).
Please note that the Pay Pal process works well, but if you pay please send in the corresponding application. WE NEED BOTH.
Additional Required Material
You are not finished quite yet. In addition to the applications, each participants must complete and sign the Football & Cheer Participants' Code of Ethics.
