2021 Registration

Hello,
Welcome to the 2021 Season. We will be holding registrations June 9th, July 14th and July 28th at World Series Baseball Park 25 Dow Street behind the Belmonte Elementary School between 5:30 pm - 7:30 pm.
Please bring/fill out the following forms (all forms printed must be single sided):
- Current PW Physical Forms (section I filled out by parent/guardian, section II filled out by medical provider)
- 2021 Participant Contract & Parental Consent Form
- Code of Conduct
- Original Birth Certificate with raised seal or Certified Copy
- COVID Waiver
- 2021 Bullying-Policy
- 2021 Concussion Policy
- 2020 - 2021 Report Card
- Copy of Medical Insurance Card
Registration will remain $275 per player which includes $50 calendars, $200 for the second child and $100 for the 3rd and may be paid by check or through Venmo @Saugus-PopWarner. All fees are due by August 1, 2021. EARLY BIRD REGISTRATION is $200 if paid in full by July 14th.
