Parents, In order to streamline the process for League ID Certification we will be using the online Verification system National Sports ID. You will simply go to www.nationalsportsid.com and create an account to add your child/player. (This will save you a trip to the DMV)
- · Upload one of the following documents (Birth Certificate, State ID, or Passport).
- · Upload a picture (headshot) of your child
- · Upload report card or screenshot of school schedule
- · Pay fee.
- · Once verified you NEED TO PRINT OUT AND BRING TO COACH LATIMORE OR A TEAM MOM
- · Bring a copy of a CURRENT Sports Physical
If you currently have a National Sports ID from last season it MUST BE RENEWED.
