| Even if you have handed in your competition release forms and USASF packet I need a COPY of everyones Birth Certificate. The Birth Certificate needs to be attached to the USASF form when I send it in and these need to be sent in by 11/4/11.
PLEASE bring in a copy of your child's Birth Certificate this week.

| I also need all of the competition release forms filled out completely and turned in this week. They are 2 weeks past due and out 1st competition is in less than 3 weeks. Your child will NOT compete without all of these forms turned in this week.
