Team Roster Form (To Print and Fill Manually)
Player Name, Address, Phone, Birthdate, School, Grade, Parish, Eligibility #, and Signature required for the entire team.

Player Contract/Player and Parent Code of Conduct (To Print and Fill Manually)
Player Contract and Code of Conduct FILLABLE Form
To be held by the parish coordinator for each player on a team. When asked by the CYO Committee to provide this contract for each player on a team, please do so as instructed.
Coaches Code of Conduct (To Print out and Fill Manually)
Coaches Code of Conduct (FILLABLE)
To be completed by November 13 by each head coach and assistant coach who is most likely to substitute for the head coach. This form is maintained by the parish coordinator for all coaches in the parish/school program. When asked by the CYO Committee to present this form, in the case of a random audit, please do so as instructed.
Gym Roster Form
To be filled out at the beginning of the season and the start of the playoffs and provided at the scorers table at the gym where your team will be playing. The gym coordinator will hold onto this form for the duration of the season and playoffs.
