Print this out and have the parent sign and give to me before first game.
Use this form for a guardian who did not approve their player online.

WSL Guardian Paper Waiver_Entire Team Form
Print this form out if your entire team did not approve their players online.
If players are showing GREEN online, then don't need this form.

Beavercreek City Waiver
This form must be printed out and signed by each guardian. This form needs to be turned in at check in before first game.
There is no alternate way to do this form. Must print it out.
