Complete Your Registration!
If you have been asked to complete your registration you will need to complete the following forms:
Player Development Program Registration Payment - Players being rostered for the 2009 Seals Player Development program choose this link to make their registration payment online.
International Clearance - Must be completed by all foreign passport carriers and foreign born players if you came to the United States after the age of 11. If you came to the United States before the age of 11 any document, e.g. school document or medical record, showing your name and date prior to your 11th birthday, can be submitted as evidence of early arrival.
PDL Amateur Registration - For all amateur players
PDL Amateur Registration - For all amateur players
Pro Player Registration Form and Pro Player Contract - For all professional players.
USASA Registration Form 2006 - For amateur players who are Glens candidates
Liability Waiver - all PDL players

No Primary Insurance Waiver - This form acknowledges that you do NOT have a primary insurance and still are willing to play soccer in the PDL i.e. you are liable for medical expenses over and above those covered by PDL secondary insurance policies.
US Club Registration Form - This form must be completed by player who were born after Jan 1, 1988.

You also need to complete the Player Resume Maker (see below), which will provide information about you for the fans.
Completed forms must be turned into the Seals as quickly as possible. Along with the appropriate forms be sure to send the following:
1. A copy of your birth certificate
2. A head shot photograph for your pass and for our website. A jpeg (or jpg), gif, or tiff file can be sent as an attachment.
For rapid processing of your registration and to verify it is complete, we recommend you scan all forms, and attach them to an email which must be sent to:

Player Resume Maker
All players please complete your online resume by clicking on the button below. Your resume will be posted online in PDF format for fans and other coaches to download and/or print. Please note, that all data will appear. Therefore, if you put your email address in the resume, it will be available for ANYONE to email you.
Be prepared to complete the resume in one sitting. Unfortunately the system will not allow you to update your form at this time. To view a sample of the questions on the Resume Maker, view the Sample Resume for C.J. Brown, who was a former San Francisco Bay Seals player.

Player Ticket Package
As we get nearer to the season you will be able to download your Player Friends & Family ticket package.
This ticket package will allow you to distribute vouchers that will allow your friends and family to purchase tickets at a substantial discount.
