Insurance Policy for 2023-24 Soccerland Season

 

The Player Insurance Coverage for the 2023-24 Soccerland Indoor season is OPTIONAL at the time of individual player registration. Those who wish to OPT INTO the insurance plan will be charged a fee of $5.00, and will be eligible for coverage in the event of an injury occurring as a result of their participation in the Soccerland League at the University of Winnipeg RecPlex. Those who choose to OPT INTO the insurance coverage must etransfer $5.00 to soccerlandleague@outlook.com , using the password  insurance.

 

Those who choose to OPT OUT of the insurance coverage will be solely responsible to cover any and all expenses associated with their loss, damage, or injury that may occur as a result of their participation in the Soccerland League at the University of Winnipeg RecPlex. The Soccerland League, the University of Winnipeg RecPlex, and their agents are free of any and all future claims, and you are relinquishing your right for any compensation as a result of your participation during the 2023-24 Indoor Season.

 

** The insurance plan enrollment cannot be backdated, and the choice to enroll or not must be made during the initial player registration process. **

 

 

** All claims and initial emails must be received by the Claims Adjustor within 30 days of the incident occurring, otherwise they will be unable to process your claim. **

 

 

 

 

 

-          In the event of an injury, you and your witness(es) MUST fill out the forms at the U of W RecPlex the evening of your injury. Any of the staff members present can guide you on what is all required, and the Claims Adjustor will be receiving them as part of your potential claim.

 

-          Once an injury occurs, please email soccerlandleague@outlook.com to receive the insurance paperwork necessary for you to start your claim, and so that we can ensure that our portion of the paperwork can be completed within the required time frame. Once we have received the intention email, we can forward the paperwork and contact details for the insurance company to the injured player.

 

 

-          Certain aspects, such as an ambulance bill, physiotherapy, dental, medical aids (crutches, air cast, brace) are covered to a pre-set amount. The insurance company must receive all of the primary physician’s prescriptions and the receipts for everything in order to add it to your claim. All braces and aids must be prescribed for daily wear and usage to rehabilitate, and will not be covered if it’s a brace or device to be worn while playing sports at present, or in the future.

 

-          In order for your chiropractor or physiotherapy appointments to be considered for reimbursement, you must submit your primary physician’s prescription for each of the specialized services. If you choose to see a chiropractor, massage therapist or physiotherapist on your own, they will NOT reimburse you any of the costs associated with those visits.

 

 

-          You must submit any and all claims to your personal insurance company first (through your work, if you’re covered under your parents or spouses’ work, and so on), and you will need to submit that paperwork to the League’s claims adjuster. They will need to see what your primary insurance company has or will be covering before the League’s plan will take effect. (the League Insurance is a secondary insurance plan for each of the members)

 

-          The League insurance company may choose to reimburse the player after all of the prescriptions, referrals, and receipts have been received and processed, instead of sending out payments sporadically for only a few bills at a time. They do not allow for direct billing, and there could be a couple / few / or more months wait to receive full reimbursement in between the date of the accident and the date that the cheque or payment will be sent to the player. The League has no control over the payout schedule, and once we have submitted our information, the Claims Adjustor will deal solely with the injured player, and on their timeline.
