WAIVER OF LIABILITY FORM: Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19 The Moon Youth Wrestling are navigating through this season in a well thought out process; however, there are still potential risks in this environment of COVID-19. We ask you as wrestlers and parents to stay home if not feeling well or have had contact with known or suspected COVID-19 individuals. We are going to do our best to keep Moon Wrestling practices and facilities a safe place but there are always unknown risks. We are asking you to sign the below waiver to protect those organization trying to ensure this season happens. The novel coronavirus (“COVID-19”) has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. Moon Youth  Wrestling have put in place preventative measures to reduce the spread of COVID-19; however, cannot guarantee that you will not become infected with COVID-19. Further, attending any related program or activity could increase your risk of contracting COVID-19.By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 while attending Moon Youth Wrestling practices and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Moon Youth Wrestling  volunteers, program participants, and their families. I voluntarily agree to assume all the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at Moon Youth Wrestling practices/events. On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless Moon Youth Wrestling, its volunteers, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Moon Youth Wrestling, its volunteers, employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after attending any Moon Youth Wrestling program or activity. 


PARENTAL CONSENT: I am the minor’s parent or guardian named above and I understand the nature of the Waiver of Liability above and verify and consent to the minor attending Moon Wrestling events. On the minor’s behalf, I hereby release, covenant not to sue, discharge, and hold harmless the Moon Wrestling, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. On behalf of the minor, I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Moon Wrestling, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after attending in any Moon Wrestling program or activity. Parent/Guardian Approval, Release, and Medical Release Recognizing the possibility of physical injury associated with wrestling and in consideration for the Moon Wrestling and its affiliates accepting the registrant for its programs. I hereby release and/or indemnify the Moon Wrestling, its affiliated organizations and sponsors, their employees and associated personnel against any claim by or on behalf of the registrant as a result of the registrant’s participation in the Program and/or being transported to or from the program. My child has received a physical examination by a physician and has been found physically capable of participating in this program. Incase of accident or serious illness, I request the Moon Wrestling to contact me. If Moon Wrestling is unable to reach me, I hereby give my consent to have a coach and/or Doctor of Medicine or dentistry provide my child with medical assistance and/or treatment and agree to be responsible financially for the reasonable cost of each assistance and/or treatment. I release the Moon Wrestling and their coaches of liability in case of accident or injury. 


A Little About Wrestling

Wrestling is the Fairest Sport for EVERY child

Kids will wrestle other kids of approximately the same age and weight. For example, in Youth Wrestling, the age categories are 6 and under; 8 and under; 10 and under; 12 and under. Ages for tournaments are generally determined by the child's age as of 1/1 of that current seasons year. Weight classes are in five pounds increments (for example the 65 pound weight class would include kids from 61 to 65 pounds). It assures the kids only wrestle against kids their size. In tournaments you will find 3 different types: 1st year, Novice, and Open. Novice Tournaments are for kids with only 1 to 2 years of experience, where an Open Tournament is for kids with 3 or more years.

 

About our Program...

Our youth program practices are divided by experience level to ensure your wrestler gets the best training to improve his/her skill set. Experienced wrestlers (3rd year and above or invited novice) practice every Monday and Wednesday from 6:30 pm to 8:00 pm. Novice wrestlers (1st and 2nd year wrestlers, along with hand selected other wrestlers) practice every Tuesday and Thursday from 6:30 pm to 8:00 pm. Head Coach Sean Thomas will lead instructions in the room, and assistant coaches are there to promote learning and safety. Coaches will meet and plot out what skill sets each group needs to bring the most positive experience. Our program is about teaching the sport and skills. Our weekly wrestling meets are on the weekends, generally spanning from the beginning of January till the end of February, ending with the WAWA Novice and Open Tournaments.
