To register for the Seals U20 Team Tryouts, follow each instruction below:
2. Complete the Seals US Club Soccer U20 Registration & Medical Form
Print this form, complete it, sign it, and mail to the Seals Address listed below, or bring it to the tryout.
3. If you do not have health insurance you must fill out the No Insurance Waiver Form
Print this form, complete it, sign it, and mail to the Seals Address listed below.
4. Pay One-time 2007 Season Tryout Fee of $25
Print this form, complete it, sign it, and mail to the Seals Address listed below, or bring it to the tryout.
3. If you do not have health insurance you must fill out the No Insurance Waiver Form

4. Pay One-time 2007 Season Tryout Fee of $25
- Make your Tryout Registration Payment on-line with your credit card or PayPal account
(Note: If you were rostered with the Seals PDL or U20 in 2006, or you already paid for the 2007 PDL tryout, you do NOT need to pay this year)
or - Complete this Payment Form and mail payment
5. Once you have completed the above steps, mail any printed forms and a copy of your birth certificate to:

- Note: If you were NOT ROSTERED LAST YEAR you must include a copy of your birth certificate with your paperwork.
- You may bring completed paperwork to the first tryout you attend.
