Southeast Health will be visiting Advance High School for student-athlete physicals on August 4th beginning at 9am. Please enter at the Superintendent entrance.
Physicals will cost $25 each. They will accept cash(please bring exact change) and checks. Please make checks payable to: Southeast Missouri Hospital Physicians
All students and parents will need to fill out the MSHSAA Pre-participation Documentation - Annual Requirements.
Please contact your coach or Mr. Hamlin: jhamlin@advancehornets.org if you have questions about when your physical expires.
Parents and students whose physicals are due will need to fill out the MSHSAA Pre-participation Physical Evaluation Form. These forms are available at the school offices, or via the links below.
