Cheerleaders, Basketball, & Volleyball players DO NOT pay your athletic fee until after tryouts

Final Forms is an online system that will allow you to register your student - athlete and complete most required forms electronically, saving you time and less paperwork.  The only form that is still required to be handed in is the actual OHSAA Physical Form that the doctor will need to sign.  
They must be completed prior to the start of tryouts or practice. Do not pay the online athletic fee if you are trying out for a team. Please pay after tryouts. Click on the link below for FinalForms
