WAIVER INFORMATION

1. I, the parent/guardian of the above-named candidate for a participation in the Long Island HellCats Baseball/Softball Winter Training Program, hereby give my/our approval to participate in any and all related activities, including transportation to and from such activities.

2. I fully understand that participation in the sport of baseball or softball may result in serious injuries and/or death and protective equipment does not prevent all injuries to players. I/We do hereby waive, release, absolve, indemnify, and agree to hold harmless Long Island HellCats Baseball, Inc., its Officers, board members, coaches, player and other volunteers, all facility owners/managers including OBEN School district, Frozen Ropes, Cove Sports Academy, trainers/players from Molloy College, CW Post and SUNY Old Westbury from any claim arising out of any injury and/or death to my/our child.

3. I agree to pay any and all fees associated with my/our child’s(rens) participation in the Winter Training Program in full prior to the first training session. Such fees will be paid in cash or check made out to Long Island HellCats Baseball, Inc.
