Parent Agreement

I/we, _____________________________, the parent(s)/guardian(s) of __________________________ will be part of 2013-2014 Diamond Hawks Fast Pitch Program. I/we understand that this is a competitive, “A” level club team. The position(s) that my/our daughter will play, and the playing time that she receives will be determined solely on a competitive situation and my players coach’s assessment of what would be in the best interest of the team. I/we also understand that though my/our daughter has earned a place on the roster, I/we must fulfill my/our responsibilities to the organization.
AS A PARENT OF THE DIAMOND HAWKS FAST PITCH PROGRAM, I/WE WILL: (Please initial after each statement)
1. Respect all Coaches, Parents, Players, Team Staff, Tournament staff and officials ______
2. Participate in practices, games and any other team activities as scheduled and provide advanced notification via email or phone call if less than 24 hours of any schedule conflicts ______
3. Have a positive attitude and support our team during the season whether my/our player is playing or not ______
4. Display good sportsmanship at all times ______
5. Commit and participate 100% ______
6. Fulfill our financial commitments to the program _______
7. Keep in mind that this is not a recreation program. The best defensive and offensive team will be fielded for tournaments based on stats, playing performance, overall knowledge of the game and position needed ______
8. Once contracts are signed (Player Agreement and Parent Agreement), the Diamond Hawks Program fee will not be refunded regardless of reason for termination _______
9. Will not approach the bench during games and tournaments unless medically necessary or in case of an emergency ______
* In the event that one of or multiple of the above have been broken, the player will be sat for the entire next game following the incident. In the event this has been broken twice, on different days, the player will be dropped from the original roster and there will not be a refund ______
