WAIVER INFORMATION

I, the parent/guardian of the above named child who is a member of the Jr. Knights Youth Football and Cheerleading Association, hereby give my approval and consent for my above named child to participate in any and all youth football and/or cheerleading activities, including transportation to and from the activities. I am aware that participation in football and/ or cheerleading could result in physical illness, serious injuries and/or death, and protective equipment does not prevent all injuries to players or cheerleaders. I, in my own behalf and in behalf of my above named child, acknowledge that myself and my child are assuming the risks of such illness or injury by participating in this sport. In the event of such illness or injury, I authorize the Junior Knights Youth Football and Cheerleading Association to obtain necessary medical treatment for my child and hereby, in my own behalf and on behalf of my child, do hereby waive, release, absolve, indemnify, and agree to hold harmless the Borough of Sharon Hill, the Sharon Hill Ramblers Athletic Association, the Junior Knights Youth Football and Cheerleading Association, involved coaches and/or teams, the organizers, sponsors, participants and persons transporting my child whether result of negligence or for any other cause. I further understand that I will be responsible for any and all medical and related bills that may be incurred on behalf of my child for any illness or injury that my child may sustain during their participation in football or cheerleading and while traveling to and from Jr. Knights functions or to and from the site of a game, whether or not the game actually occurs.
