WAIVER INFORMATION

I the parent or guardian of the above player (or player if of legal age), hereby consent to her (my) participation in the activities or the Barrie & District Girls Softball Association (known as the BDGSA).
I hereby waive my right of action, causes of actions, suites, debts, covenants, contracts, claims and demands whatsoever which I may have or which my heirs, administrators, or assigns, or any of them hereafter can, shall, or may have for or by reason of or any cause, matter, or thing whatsoever arising out of said activities of the BDGSA or travelling to or from any such activity. And I covenant to save harmless and indemnify against any matter of actions, causes of action, suites, debts, covenants, contracts, claims, and demands made against the BDGSA by reason of any acts or omissions of the said Player causing damage to any property or injury to any person.
I also hereby agree to abide by the rules of play of the BDGSA and any decisions of the BDGSA regarding rules of play, player eligibility, and team and division placement of players.

MEDIA RELEASE

As part of a vibrant club we like to promote team activities and celebrate member achievements. For example, we might make a Facebook, Instagram or Twitter post featuring your daughter and/or team.

I grant permission for the Barrie and District Girls Softball Association to use and publish details, including names, event details and outcomes, photographs’/images, audio or video footage of myself or my child/ren for promotional purposes on the BDGSA social media, online channels (website, Facebook, Twitter, Instagram etc) and other media outlets.

I understand that any details, images, video or audio taken may be seen in a public environment.

I acknowledge that BSGSA is not obliged to include my child in any related promotional activities.

I understand that I am responsible for paying two additional installments on our before Jan. 31, 2019 ($200) and Mar. 31, 2019 ($200) unless I have paid in full at the time of registration or have made approved arrangements with the BDGSA Executive *
