We, the parent/ guardians of the above named child, give our permission and approval for his/her participation in any and all activities during the football season. We assume all risk and hazards incidental to such participation including transportation to and from all activities: and do hereby waive , release, and otherwise hold harmless the local football association, association officials, sponsors, supervisors, coaches and other participants and persons transporting our son/daughter. We the parents/ guardians also agree to return the uniform and or other equipment issued in the same condition as received , with exception of wear experienced through normal use.

The American Youth Football Houston Conference (AYFHC) operates the league on money raised by registration fees and fund raising projects such as sponsorship and donations. We do not receive any money or support from local municipalities of governments other than what is provide to the local community in the way of playing fields and facilities. Your signature assumes responsibility on your part for specified fees and to assist the league in its’ fundraising efforts.

The insurance provided through AYFHC and AYF is paid for out of the funds raised. AYFHC will not pay deductible. It is a secondary policy when you have other medical or accident insurance, but is primary coverage if the above named child is not otherwise insured. You should be aware that in spite of all reasonable precaution, injuries could happen. Football is a collision sport, and even the best equipment will sometimes not prevent injury. Money paid at registration will not be refunded unless your child does not meet qualification of age/grade matrix and / or medical release.

DUE TO THE RECENT OUTBREAK OF COVID 19 ALL PARTICIPANTS ARE PARTICIPATING AT THEIR OWN RISK. BY SIGNING THIS FORM I ACKNOWLEDGE THAT GARDEN VILLA GIANTS, AYFHC, AYF, SADLER INC AND ANY ENTITY, TO INCLUDE BOARD, COACHES, STAFF AND GAME OFFICIALS, ASSOCIATED WITH THE PARTICIPATION OF YOUTH SPORTS DURING THE 2020 SEASON IS NOT LIABLE FOR THE CONTRACTION OR SPREAD OF ANY THING COVID RELATED.
