I/We the parents or Guardian of the above listed applicant hereby give my approval for his/her participation in any and all Farmersville Pee Wee Football/Cheer activities.

I/We assume all risks and hazards incidental to such participation including transportation to and from the activities and I/We hereby release, absolve, indemnify and agree to hold harmless the Farmersville Pee Wee Football/Cheer Association, organizers and sponsors which the Association may partner with except to the extent and in the amount covered by accident or liability insurance.
