ACKNOWLEDGEMENTS
Volunteer Acknowledgment
Honey Grove Youth Sports is an organization based on volunteers; from commissioners to coaches to gameday operations. Without the efforts of volunteers, the organization could not provide these opportunities for the youth in our community.

I, as the parent or legal guardian registering a player for the Honey Grove Youth Sports Softball/Baseball Program, acknowledge that for each player registered, one adult parent, legal guardian, or designated family member must volunteer in some capacity, for which I will select below, during this basketball season. Additionally, I understand that failure to fulfil this commitment may lead to higher registration costs and/or additional fees for future registrations in Honey Grove Youth Sports.

Waiver
This is to certify that I, as the parent or legal guardian registering a player for the Honey Grove Youth Sports Softball/Baseball Program, hereby grant permission to the adult manager, coach, trainer, or business manager of the team to obtain medical care and emergency dental care, at my expense, from any licensed physician, hospital, or medical center, for the player registered above, at such time as either parent or legal guardian cannot be contacted in person or by telephone. This authorization shall include all league activities, including the period required to travel to and from those activities; and I do hereby waive, release, absolve, indemnify, and agree to hold harmless HONEY GROVE YOUTH SPORTS, its board members, coaches, assistant coaches, sponsors, participants, and persons transporting the player to and from those activities, for any and all claims arising out of an injury to the player.

Player/Parent Codes of Conduct
I acknowledge that I have access to, have read, and will abide by, the Parent and Player Codes of Conduct found on Honey Grove Youth Sports website, http://www.leaguelineup.com/honeygroveyouthsports, or on the Honey Grove Youth Sports' Facebook page. I understand that if I am unable to access above the mentioned Codes of Conduct, I may request a copy via email at honeygrovedixieleague.com. Further, I acknowledge that both myself and my player's other parent(s), family members, or other spectators are required to abide by the Codes of Conduct as well.

Photographic Consent & Release
I, as the parent or legal guardian registering a player for the Honey Grove Youth Sports Softball/Baseball Program, hereby authorize Honey Grove Youth Sports (HGYS), and those acting pursuant to its authority to: Record my child's likeness and voice on a video, audio, photographic, digital, electronic, or any other medium, use my child's name in connection with these recordings, and use, reproduce, exhibit or distribute in any medium (e.g., print publications, video tapes, HGYS websites and social media accounts) these recordings for any purpose that HGYS deems appropriate, including promotional or advertising efforts. I release HGYS and those acting pursuant to its authority from liability for any violation of any personal or proprietary right I may have in connection with such use. I understand that all such recordings, in whatever medium, shall remain the property of HGYS. I acknowledge that I have read and fully understand the terms of this release.
