WAIVER INFORMATION

PARENT/GUARDIAN AUTHORIZATION AND WAIVER OF LIABILITY

In order for the Ohio Thunder Fastpitch Organization team to accept your registration and permit participation in the Ohio Thunder by the named individual, I the parent or guardian of said individual hereby give my consent and agree to release, indemnify and hold harmless the Ohio Thunder Organization, it’s coaches and representatives, from any claim arising out of injury to the above named individual except to the extent and in the amount covered by any liability insurance held by the Ohio Thunder Fastpitch Organization. I also agree to hold harmless the Ohio Thunder Organization, it’s coaches and representatives from any claim arising out of injuries or conditions caused or aggravated by my refusal to obtain available medical treatment.
