I hereby understand that the Temple Youth Baseball Association, it's sponsors, organizers, supervisors, and coaches, neither individually nor collectively, assume liability whatsoever in case of accident,injury, or death sustained by or to any player or players, or spectators who may be involved in said event before, during, or after any practice or game sponsored by this organization.

I fully agree that this is a game played with a baseball and bats and that there is a certain risk involved that may result in injuries, both minor and severe, to player(s) and/or spectators. I also acknowledge a certain amount of responsibility in the prevention of accidents on behalf of myself and the above listed player. (Example: Paying attention during all aspects of the game and parents not distracting the player by coaching from the stands.)

Futhermore, I will as a player and/or parent conduct myself in a sportsmanlike manner at all times, especially when participating in any practice or game scheduled by TYBA. As a parent, I will conduct myself also in a sportsmanlike manner at all times and will also ask other spectators to conduct themselves in a like manner. I understand that unsportsmanlike conduct will not be tolerated and that I may be ejected from the game or field(s) where the games are being played.
