Benbrook Youth Baseball and Softball Association (BYBSA) is requiring each Coach to provide their Date of Birth and Driver's License numbers on all applications submitted to perform a criminal history check. This information will be used only for the purposes of obtaining a criminal history record from the Texas Department of Public Safety. The information you provide will not be used for evaluating your qualifications, however, BYBSA will consider your relevant criminal history in determining your eligibility for Coaching or Managing a team within or associated with BYBSA.

By submitting this application I understand and accept that a criminal history record will be required and I release BYBSA, its Board of Directors, and any Volunteers from any liability which may or could result from furnishing the information requested

Benbrook Youth Baseball and Softball Association has the right to refuse any Coaches application.

. Every coach will be responsible for the sportsmanship of his/her players.
. Every coach will be responsible for assuring that parents/adults are available for concession duty during the allotted time slot for your team.
. Every coach will be responsible for adherence to all League rules, including those concerning sportsmanship and player participation. UNSPORTSMANLIKE CONDUCT WILL NOT BE TOLERATED!
