AMYSA / A&M STEELERS MEDICAL RELEASE / LIABILITY WAIVER

As Parent / Guardian of the above mentioned child. I hereby give my approval for participation in any and all "Arlington Mansfield Youth Sports Association" (herein called the AMYSA) activities during the 2025 season.

I hereby waive, release, indemnify, and agree to forever hold harmless the A&M Steelers: Sponsors, Directors, Coaches. Participants and other AMYSA officials from responsibility including but not limited to my child, and/ or myself, while participating in any and all A&M Steelers activities. Furthermore, I will inform any guest and/ or family that they are entering any and all A&M Steelers events at their own risk.

I give my permission for the AMYSA Steelers Y.S.A and their designated officials to seek such medical assistance as required in the event of injury to my child while participating in any AMYSA activity. I will provide a copy of my Child's Birth certificate and a current photograph to the AMYSA Steelers for identification and insurance purposes prior to the season commencing. The parent / guardian (s) of the child must sign agree to this form.
The AMYSA Steelers is a non profit entity and there is a no refund policy, after 5 business days.
Each child must complete the program to receive an award at the end of the season.
