PLAYER INFORMATION
CYO participants are required to be registered OLP/ND parishioners or students of the school.



Parent Awareness Acknowledgement
On behalf of the above applicant both parents and other guardians and collectively with you named on this form, hereby grant permission for the youth's participation in all activities of the Notre Dame de Lourdes CYO Sports Program, and also assume responsibility for all risks and hazard incidental to the youth's participating including travel to and from activities. The undersigned waives, releases, absolves, indemnifies and agrees to hold harmless Our Lady of Peace d.b.a Notre Dame De Lourdes CYO (NDDL CYO), its coaches, administrators, its sponsors and persons providing transportation for youth's travel to and from an activity for any claim or liability arising out of an injury to the youth or out of any loss suffered directly or indirectly while participating in or travelling to or from an activity of the Notre Dame de Lourdes C.Y.O.

Additionally, both parents and other guardians acknowledge that they have read and agree to abide by the Parental Code of Conduct described below and will review and communicate to your child their responsibilities as set out below in the Child Code of Conduct.

PARENTAL CODE OF CONDUCT
- ZERO TOLERANCE FOR BAD BEHAVIOR!
- As a parent/guardian/coach I will make sure that my child follows the Players Code of Conduct below
- As the parent/guardian of a child in NDDL CYO program, I will follow the Parental Code of Conduct
- I understand that negative behavior that disrupts the child's practice, game or sports season will result in a decision from the CYO Board. I understand that unruly behavior will result in my family's removal from NDDL CYO and its facilities used during an NDDL CYO event.
- Please note this includes social media posts and group emails/texts to other parents/coaches or league.
- I will discuss with my child the importance of good sportsmanship and the penalties should my behavior be considered BAD BEHAVIOR.

CHILD CODE OF CONDUCT
- I will encourage good sportsmanship from fellow players, coaches, officials and parents at every game and practice by demonstrating good sportsmanship.
- I will attend every practice and game that I can, and will notify my coach if I cannot. I will be on time for practice and games.
- I will do my best to listen and learn from my coaches.
- I will treat my coaches, and other players, officials, and fans with respect regardless of race, sex, creed and abilities. and will expect to be treated accordingly.
- I deserve to have fun during my sport experience and will alert parents or coaches if it stops being fun.
- I deserve to play in an environment that is free of drugs, tobacco and alcohol and expect adults to refrain from their use at all youth sports events.
- I will encourage my parents to be involved with my team in some capacity because it is important to me.
- I will learn the rules of the game.
- I will not engage in unsportsmanlike conduct with any coach, parent, player, participant, league official or attendee.
- I will not engage in any behavior which would endanger the health, safety, or well-being of a coach, parent player, participant. league official or any other attendee.
- I will not engage in the use of profanity while attending or participating in an NDDL CYO sports event.
- I will not engage in verbal or physical threats of abuse aimed at any coach, parent, player, participant, league official or attendee.
- I also agree that if I fail to abide by the above code of conduct, OR if my guardian fails to abide by the above code of conduct, I will be subject to disciplinary action that could include but limited to removal of all CYO activities.
- I have discussed BAD BEHAVIOR and the code of conduct for parents and child. I will demonstrate good sportsmanship no matter the circumstance and understand the penalty if I don't.
