PLAYER INFORMATION

PARENT/GUARDIAN

Please indicate if you will be trying out as a pitcher and/or catcher. We encourage anyone with pitching or catching experience to show this at tryouts, even if it is not your primary position.


IMPORTANT INFORMATION - PLEASE READ AND ACKNOWLEDGE

The Jr High Club program is intended to give girls the opportunity to play together with other girls from Little Miami and to prepare for the HS softball experience. As such, decisions on rosters, team splits, etc are made with the best interest of the overall program in mind and with guidance from the HS coaches. If fielding multiple teams, players will be assigned to a specific roster on which they are expected to play.

As this is a competitive program, not all girls that try out will be selected for a team. Further, not all girls necessarily receive equal playing time. Playing time is earned via performance in practices/games and is fully at the coach's discretion.

There is a fee to play for the Jr High club team. Final costs are determined after rosters are set but it is typically around $200/player depending on final roster sizes.


Waiver Agreement
IN CONSIDERATION OF my child/ward, being allowed to participate in any way in the LITTLE MIAMI FASTPITCH ASSOCIATION related events and activities, the undersigned acknowledges, appreciates, and agrees that:

1. The risk of injury to my child/ward from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,

2. The risk to have contact with individuals, who have been exposed to and/or have been diagnosed with one or more communicable diseases, including but not limited to COVID-19 or other medical conditions, diseases, or maladies does exist, and it is impossible to eliminate the risk that I could be exposed to and/or become infected through contact with or close proximity with an individual with a communicable disease;

3. I FOR MYSELF, SPOUSE, AND CHILD/WARD, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my child/ward's participation; and,

4. I willingly agree to comply with the program's stated and customary terms and conditions for participation. If I observe any unusual significant concern in my child/ward's readiness for participation and/or in the program itself, I will remove my child/ward from the participation and bring such attention of the nearest official immediately; and,

5. I for myself, my spouse, my child/ward, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS LITTLE MIAMI FASTPITCH; its directors, officers, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event, WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property incident to my child/ward's involvement or participation in these programs, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

6. I, for myself, my spouse, my child/ward's, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY INDEMNIFY AND HOLD HARMLESS all the above Releasees from any and all liabilities incident to my child/ward's involvement or participation in these programs, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
