> In order to reduce equipment losses and to maintain our low registration fees, every one of WYA football players will now be required to:
> 1. Submit a mandatory $50.00 equipment deposit due upon receipt of leased football equipment. A check or money order, payable to WYA will be held on file and returned to you when the leased equipment is returned.
>
> 2. If you fail to return the leased equipment your check will be cashed and you will be billed for the replacement cost of the football equipment.
>
> Football Equipment Lease Agreement: By accepting the terms here, I, the undersigned parent(s) or legal guardian(s), hereby guarantee the safekeeping and timely return or reimbursement of the replacement value of any and all equipment issued to my child by the Warrior Youth Association. I will not modify or mark the equipment in any manner to make it unusable or undesirable for future participants. I have received in good condition a helmet with chin strap and shoulder pad. Equipment shall be returned as issued, excepting for normal wear and tear, clean and in good condition to WYA.
> Furthermore, I, the undersigned parent(s) or legal guardian(s), understand that failure to return any of the equipment issued at the end of the season will result in the forfeiture of my $50.00 equipment deposit and the issuance of a minimum $150.00 invoice for the replacement cost of the leased football equipment. Furthermore, I understand that my failure to either return said equipment or reimburse WYA for the missing equipment will preclude my player from participating in any future WYA activities. Additionally, I understand that legal actions may be taken against me to recover either the leased football equipment or the replacement cost of the retail value of said equipment and I will be responsible for WYA's attorney fees to recover the funds.

> In consideration of the Warrior Youth Football Association allowing the above-named player to participate in its football program. I do hereby release and agree to hold the Warrior Youth Football Association, their officers, officials, directors, agents, members, employee/coaches thereof harmless and from any liability for injury to the above-named player which may result directly or indirectly from participation in this football program. I believe the above-named player to be in good physical health and fully able to participate in the football program. Further, I hereby agree to authorize any representative of the Warrior Youth Football Association, its officers, members, or agents to render first aid and secure medical treatment and transportation as my representative if, in the sole opinion of said officer, member or agent such treatment appears necessary of desirable for wellbeing of said player
