WAIVER INFORMATION

For the 2019 Spring HS Rattlers-Berneche - Baseball Team.
I understand that my son’s participation within Flower Mound Youth Sports Association / Texas Rattlers-Berneche Baseball Team shall be restricted to this team until my son is released by the Head Coach, or until this season concludes (whichever occurs first.). I also understand, that my son’s participation with any other team must have written approval from the Head Coach of the HS Texas Rattlers-Berneche, as not to violate any league roster rules “FMYSA, USSSA, Super Series, TEBA, Nations Baseball etc…”, while still a member of the above team, would be a rule violation, and that both my son and the other team would be subject to disqualification and suspension. I also agree and aware if my son leaves the team prior to end of 2019 season I will forfeit any monies paid to the team for tournaments, cage rentals, equipment, private lessons, uniforms.
Required $150.00 Deposit due by Feb. 16th.
