Please provide your company information below.

Your logo should be sent in a JPEG, PNG or PDF format to frederickbaseball.cloverhill@gmail.com. Once registration is complete, your company logo will be posted on the Frederick Baseball Clover Hill website (FBCH.net) along with the company URL that you provide.

The logo may also be used for promotional materials or emails that are distributed by Frederick Baseball Clover Hill.


CONSENT FOR PARTICIPATION AND RELEASE OF LIABILITY:
I give permission for our company to be acknowledged as a sponsor for Frederick Baseball Clover Hill located in Frederick, Maryland and do hereby acknowledge and recognize that our company logo and website will be linked to the FBCH.net website, advertised in the Clarion Newsletter, and the company name may be used on brochures and/or any marketing material for the use of promoting the league and league events.

Platinum Level: $500.00


• Your company name will be on the arm of one of our team's jerseys based on availability of teams.
• A 5 x 7 plaque of appreciation from Frederick Baseball Clover Hill with your team's picture.
• Your company will have a sign, approximately 3’x4’ in size, which will hang on the outfield fence of our home field.
• Your company will be listed as a sponsor on our website and will have a link from our website to yours.
• The opportunity to come to our Spring Opening Day Ceremonies to be introduced to the players, friends and family as a sponsor of Frederick Baseball Clover Hill.
• The opportunity to come to our year end picnic and pass out the trophies to your team.
• We will provide your company with a space to set up a table to hand out literature and/or promotional products at both the Opening Day Ceremonies and the picnic.

Gold Level: $325.00

• Your company will have a sign, approximately 3’x4’ in size, which will hang on the outfield fence of our home field.
• Your company will be listed as a sponsor on our website and will have a link from our website to yours.
• The opportunity to come to our Opening Day Ceremonies to be introduced to the players, friends and family as a sponsor for Frederick Baseball Clover Hill.

PAYMENT:

You may complete your payment on-line at the time that you complete your registraton. There is an additional fee to complete payment on-line.

BY MAIL
After completing your on-line registration payment may be sent by mail. Checks should be payable to CHCA and sent to:

Please note that any returned check will incur a $35.00 returned check fee. Registration will not be considered complete until full restitution is made.
