I hereby give permission for my child/children to participate in the Christian Athletic Association. I accept full responsibility for any equipment and/or uniform required for the season and return of that equipment and uniform if required. I understand that there will be no registration refunds. I will not hold CAA, coach, or facility of participation responsible for injuries acquired due to the nature of the sport.

The fall baseball fee is $90. This covers hats, shirts, umpire fees, equipment fees, and field use/maintenance. Please make check out to “CAA”. Please send/drop off this form with payment to the following places:
