SMHA Coaching Application Form 2023-2024

All forms MUST be accompanied by a current Criminal Record Check (CRC) and Vulnerable Sector Check (VSC) at the time of submission to be considered. Any forms without a CRC/VSC will not be accepted, unless you have confirmed SMHA has a recent one already on file. All information collected will be kept confidential within SMHA.

This is a requirement for ALL volunteers of HNB 18 and over every 3 years (as per section 4.0 of the minor ops manual).

Note: volunteers renewing their CRC may use the mybackcheck option; however, this does NOT cover the vulnerable sector check.
