Washington Twp Baseball will provide each player a jersey, hat and socks. Each player is responsible for their own gray pants.

WAIVER INFORMATION

Having been informed of the organization of Washington Twp Baseball to provide supervised baseball games for boys and girls, I/we, the parents or guardians of the above name candidate, do hereby give my/our consent to his/her participation in any and all activities during the current season. I/We do assume any and all risks and hazards incidental to the conduct of the activities, transportation to and from any activities and I/we do further hereby release, absolve, indemnify and hold harmless the Washington Twp Baseball Organization, the organization sponsors, and the supervisors, and and all of the organizers, the sponsors, and or all of them. In case of injury to my son/daughter, I/we hereby waive all claims against any or all of the organizers, the sponsors, and the sponsors appointed them. I/We likewise release from any responcibility any person transporting my/our son/daughter to or from the activity(s).

I/We are in the position to furnish, upon request, by league officers and/or officials, a certified copy of the birth certificate of the above candidate.

It is the parents responsibility to ensures that their child/children have and are wearing the appropriate safety gears required at practices and games.

****Pony and Colt Teams - Please be aware that this is a competitive league and you will be played according to your skills and subject to league rules.
