Parent/Guardian Authorization and Waiver of Liability

In order for the Ohio Warriors Fastpitch team to accept your registration and permit participation in the Ohio Warriors Fastpitch evaluations by the named individual, I the parent or guardian of said individual hereby give my consent and agree to release, indemnify and hold harmless the Ohio Warriors Fastpitch Organization and Springfield Township Baseball/Softball Association, it's coaches and representatives, from any claim arising out of injury to the attached named individual except to the extent and in the amount held by Ohio Warriors Fastpitch Organization. I also agree to hold harmless the Ohio Warriors Fastpitch Organization and Springfield Township Baseball/Softball Organization, it’s coaches and representatives from any claim arising out of injuries or conditions caused or aggravated by my refusal to obtain available medical treatment
