WAIVER INFORMATION

WAIVER

In consideration of Quinlan Sports Association(QSA)allowing the undersigned participant to participate as part of QSA, I recognize that my child could incur risk in participation in this event. I hereby absolve now and forever QSA, BISD, QISD an or any organizer of QSA, BISD, QISD, the competitions, their games, their agents, employees and all other person, firms and corporations that might be involved with them, whether named or not, all and each of them of any from any and every claim, demand, action, right of action of whatsoever kind of nature, either by law or in participation in and with QSA, BISD school gym and all facilities, QISD school gym and all facilities during the period of practices and or competitions. Whether by negligence or not following the direction set by directors/coaches. I further agree to release QSA and/or its representatives from any responsibility in the event of an injury to said participant. Parent/Guardian is responsible for the $50.00 deductible for insurance in case of injury.

$25.00 Charge for all returned checks, violators will be prosecuted. QSA board meetings are held monthly the first Wednesday, following the first Tuesday. QSA board meetings start at 7pm and are open to the public.
