WAIVER INFORMATION

In participating with the Hub City Hitmen Baseball Travel Team, I understand that baseball is an inherently dangerous sport and activity, and that in our participation while competing in games, practicing, or traveling to destinations to represent the Hub City Hitmen, such participation shall be at our own risk.

I, for myself, my heirs, executors, administrators, personal representatives, successors, and assigns, do hereby release and hold harmless the Hub City Hitmen, Hagerstown Baseball Association, Justin Weaver, and their affiliates as well as field venues, and their owners, from any and all actions, causes of actions, suits, judgments, claims, and demands whatsoever, in law or equity, including, without limitation, any damages resulting from all acts of activity of active passive negligence on the part of the Hub City Hitmen, Hagerstown Baseball Association, Justin Weaver and their affiliates.

I hereby expressly release, acquit, and forever discharge the injuries, to myself, my child, or to my property I may sustain as a result of my use and association of any baseball fields and their properties, venue host of all
games and/or practices, as well as the Hub City Hitmen, Hagerstown Baseball Association, Justin Weaver, and their affiliates.

I also give permission for emergency treatment of my child, if needed and assume all responsibility for the financial responsibility of that treatment.

I acknowledge that I have read the terms and conditions of this Release and Waiver of Liability, and I further acknowledge that I fully understand the same. I am signing this Release and Waiver of Liability voluntarily and with full knowledge of its significance. I understand that this Release and Waiver of Liability contains the entire agreement between the Hub City Hitmen,
Hagerstown Baseball Association, Justin Weaver, and their affiliates and myself.
