DEADLINE: April 16, 2021 $100.00 per person

Payment shall be sent to individual Team Captains.
If you don't have a team, you will be assigned to one.

Cheque (made payable to Valhalla Mixed Slo-Pitch) or Cash - arrangements can be made with Glen Haywood for drop off

A separate Waiver form must be signed prior to the first played game. Your Team Captain will reach out to you
