2025 OSBA Registration Form

The player or coach wishing to participate in the Ontario Senior Baseball Association this season, must complete this form themselves in it's entirety.

By completing this form, you are registering with both the Ontario Senior Baseball Association (OSBA) and Baseball Ontario. Baseball Ontario appoints OSBA to collect fees on its behalf, included in the registration fee collected through this process. No additional monies is charged to the player/coach registering locally. Baseball Ontario invoices OSBA for fees collected.
Individual player/coach registration fees are collected and administered by OSBA Member Teams and not the OSBA itself. Therefore players/coaches are not entitled to receive any refunds directly from the OSBA.
To improve their communications and service Baseball Ontario would like to correspond with you via email. In order to comply with Canadian legislation relating to commercial electronic messages, Baseball Ontario is seeking your consent to communicate with you via electronic messages, including email, related to your relationship with Baseball Ontario including team selection, program, membership, promotional materials and other Baseball Ontario information. Under Rowan's Law, before any player/coach can be registered with the OSBA and Baseball Ontario, the player/coach must review the Concussion Education Resources provided by the Province of Ontario and must review the Player Code of Conduct.
