MEDICAL/EMERGENCY CONTACT INFORMATION
Participation in Little League Baseball requires the ability to run, throw and catch a baseball, as well as swing a baseball bat. Does your child have any current conditions or restrictions that limits his or her ability to perform this activity.

WAIVER INFORMATION
I/We the parents/guardian of the above named participant hereby give approval for my child to participate in any or all Little League baseball activities, including transprotation to and from each activity.I/We the parents/guardian know that participation in baseball may result in serious injuries and that proper protective equipment does not prevent all injuries to players. I/We the parents/guardian hereby waive, release, absolve, indemnify, and agree to hold harmless the local little league association, its executive board & subsequent committees, sponsors, coaches for any claim arising of any injury to my child whether the result of negligence or any other cause, except to the extent and in the amount covered by the accident or liability insurance provided. I/We the parents/guardian will return any equipment and uniforms upon request of the coach or the executive board. Failure will result in reimbursement to the league with the exception of the normal wear and tear of the equipment and uniforms. By agreeing to the terms and typing your name you are filing an online form attesting you are the parent or guardian of registrant. ANY FALSE INFORMATION PROVIDED WILL BE TRACKED AND TURNED OVER TO PROPER AUTHORITIES. YOUR ARE ATTESTING THAT YOU ARE THE PARENT OR GUARDIAN OF REGISTRANT.
ZERO TOLEANCE PARENT & FAN POLICY and ACKKNOWLEDGEMENT OF USA BAT POLICY



The Dapper Dan League has adopted a ZERO TOLERANCE POLICY. This policy applies to all coaches, players, parents, and other supporters within or outside the field of play. Abusive and obscene language, violent play, violent conduct, fighting, and other behavior (including but not limited to arguing, badgering,sarcasm, taunting, etc.) deemed detrimental to the game between the above mentioned groups will not be tolerated. It is the responsibility of the Parents and Fans to provide a safe environment for their children to play and must realize that it is a privilege not a league requirement to allow a spectator inside to watch a ball game. This policy will apply to all times prior to, during and after the game at the ball field and its immediate surrounding areas.

NO PARENT OR OTHER SPECTATOR SHALL ADDRESS THE UMPIRES OR OPPOSING COACHES/FANS AT ANY TIME!!!
This includes but is no limited to:
No spectators making derogatory comments to players of either team.
No disputing calls during or after the game.
No yelling at the umpires, including no criticism, sarcasm, harassment, intimidation or feedback of any kind before, during, or after the game.
The only allowable exceptions to the above are:
Responding to an umpire who has initiated a conversation with a parent or spectator.
Pointing out emergency or safety issues.
Penalties for Spectator Ejections:
The spectator AND THE PLAYER ASSOCIATED will be removed immediately from the facility.
The spectator will serve at least a 2 game suspension. Additional suspension from games could be added as determined by The Dapper Dan Baseball Committee and Executive Board.
The team is also subject to a forfeit as ruled by The Dapper Dan Baseball Committee and Executive Board.


For the 2018 and beyond baseball season, The Dapper Dan Club will only allowed USA standard stamp bats for play in the Major and Minor division.
