Local authorities will be contacted and appraised of any threats of physical harm against any child or adult , which can lead to an arrest and/or prosecution. *

I understand that as a volunteer for the Upland Hurricanes JAAF&C program, I agree that all information I receive and obtained from parents, players, and or staff will be kept confidential at all times. *

I agree that I will not use any website or social media to represent the chapter and/or team. *

I agree to comply with and sign a non-disclosure statement to ensure that I do not misuse information received. *

I understand that as a coach, I agree to volunteer to assist during the off-season at any events requested of me by the Executive Board of Directors so long as I receive 7 days notice. *

I understand that I will be required to work at least one shift during the Upland Lemon Festival to advertise the chapter. *

I understand that as a coach, I will be required to attend a coaching clinic. *

I agree to comply with the uniform standard (practice & games) at all times and understand that failing to comply will cause me to be removed from the field. All staff will be advised of their uniform standard at the start of conditioning. *

I understand that the uniform expense is the responsibility of each coach. *

I agree to comply with the rules and regulations set forth by the Upland Hurricanes JAAF&C as well as the SoCal JAAF Conference. *

I agree that I will receive 14 days notice of any staff meeting and will attend unless excused by the Chapter Commissioner via the Head Coach. I also understand that an unexcused absence can cause for discipline up to being removed as a volunteer. *

After submitting your application for consideration to become a coach with the Upland Hurricanes JAAF&C, you will be required to attend an interview. You will be notified and provided 14 days notice with your interview date and time. Coach's will be selected based on application and interview.
