Folcroft Boys & Girls Club Release and Waiver Agreement

Cancellation/Refund Policy:
There is a no refund policy. Continued or serious violation of the Rules and Regulations of the Club and/or
the instructions of coaches, assistant coaches and managers will constitute grounds for suspension and/or
termination of membership by the Board, WITH NO REFUND.

Photo Release:
Photographs and video may be taken at any time on behalf of Folcroft Boys & Girls Club. We may take.
pictures and or video, these may be used on our website, social media, flyers and/or Banquet Book. We will
not post names of any child in the picture. If you are adamant about not having your child's picture being
used, please notify us in writing.

Medical Release and Liability Release:

Participation in all sports can be inherently dangerous and it is impossible to ensure the safety of all.

PARENTS CODE OF CONDUCT- FOLCROFT BOYS & GIRLS CLUB

This document presents the expectations for conduct by parents at The Folcroft Boys & Girls Club. We hold athletes, parents, coaches, and community members to a high standard of sportsmanship. Enforcing this Code of Conduct allows The Folcroft Boys & Girls Club to promote a safe, rewarding, and fun sports experience for our entire community.

Please read the document carefully before signing below. Each participant is required to acknowledge the Code of Conduct before the season begins, and must abide by the Code of Conduct throughout the entire season.

Violations of the Code of Conduct may result in disciplinary action, including suspension - either temporarily or permanently as determined by the Board of Directors.
Code of Conduct for Parents

1. I will put the emotional and physical well-being of youth participants ahead of a personal desire to win.

2. I will respect players, coaches, officials, spectators, and families at all times.
3. I will respect the decisions of officials and/or coaches.

4. I will not engage in any violence, or verbal threats, or the use of any profanity.
5. I will not yell advice or instructions to my child during a game.

6. I (and my guests) will not engage in any kind of unsportsmanlike conduct with any official, coach, player, or parent such as booing and taunting; refusing to shake hands; or using profane language or gestures.

7. I will not belittle or ridicule anyone involved in a youth sporting event?in public, private, in-person, or online.

8. I will do my part to keep sports fun and positive for every youth participant.
9. I will ask my child to treat other players, coaches, fans, and officials with respect regardless of race, sex, creed or ability.

10. I will demand a sports environment for my child that is free from drugs, tobacco, and alcohol and I will refrain from their use at all sports events.
