In registering my child as a participant in the Burlington Youth Wrestling Red Devil Rumble, I understand that my child and I assume any and all risks which might be associated with this activity, and waive and release all rights and claims which my child, heirs, executors, administrators, assigns or I may have against Burlington Youth Wrestling, its directors, coaches, officials, or representatives for any and all injuries or damages of any kind suffered as a result of participation in the Burlington Youth Wrestling Red Devil Rumble.

I hereby grant permission to the BYW organization to administer first aid, secure proper treatment and/or hospitalize my child in case of emergency, provided they are unable to communicate with me in accordance with my or their best judgment.
I also give permission to Burlington Youth Wrestling to use photos of my child for publication/media purposes.
