Please complete the following form to sign your child up for the 2025 Spring Season with Tri State Girls Softball. The division your child plays in will be based on how old your child is on April 1, 2025 for the 2025 season. Returning players will remain on their previous team unless otherwise stated below.

WAIVER INFORMATION

In consideration of participating in softball and for other good and valuable consideration, I hereby agree to release and discharge from liability arising from negligence Tri State girls softball and its owners, directors, officers, employees, agents, volunteers, participants, and all other persons or entities acting for them hereinafter collectively referred to as releasees on behalf of myself and my children/parents/heirs/assigned personal representative of an estate and also agree as follows:

1. I acknowledge that softball involves known and unanticipated risks which could result in physical or emotional injury, paralysis, or permanent disability, death, and property damage. Risks include but are not limited to activities of the sport, medical conditions resulting from physical activity, and damaged clothing or other property. I understand such risks simply cannot be eliminated, despite the use of safety equipment, without jeopardizing the essential qualities of the activity.

2. I expressly accept and assume all of the risks inherent in this activity or that might have been caused by the
negligence of the releases. My participation in this activity is purely voluntary and I elect to participate despite the risks. In addition, if at any time I believe that event conditions are unsafe or that I am unable to participate due to physical or medical conditions, then I will immediately discontinue participation.

3. I hereby voluntarily release, forever discharge, and I agree to indemnify and hold harmless releasees from any and all claims, demands, or causes of action which are in any way connected with my participation in this activity, or my use of their equipment or facilities, arising from negligence. This release does not apply to claims arising from intentional conduct. Should releasees or anyone acting on their behalf be required to incur attorneys fees and costs to enforce this agreement, I agree to indemnify and hold them harmless for all such fees and costs.

4. I represent that I have adequate insurance to cover any injury or damage I may suffer or cause while participating
in this activity, or else I agree to bear the cost of such an injury or damage myself. I further represent that I have no medical or physical condition which could interfere with my safety in this activity, or else I am willing to assume and bear the costs of all risks that may be created, directly or indirectly, by any such condition.

5. In the event that I file a lawsuit, I agree to do so solely in the state where releasees facility is located, and I
further agree that the substantive law of that state shall apply.

6. I agree that if any portion of this agreement is found to be void or unenforceable, the remaining portions shall remain in full force and effect. By signing this document, I agree that if I am hurt or my property is damaged during my participation in this activity, then I may be found by a court of law to have waived my right to maintain a lawsuit against the party's being released on the basis of any claim for negligence.

Parent Code of Conduct

Tri-State Girls Softball League Code of Conduct

The purpose of this code is to establish guidelines and expectations for all who are involved in our league. This code extends beyond the players, to the coaches, parents, and guests who attend practices, games, and official functions.
We're passionate about our mission of providing the finest facility for organized girls softball and promoting teamwork, sportsmanship, safety and instruction of superior quality and value. Core to this mission is ensuring the coaches and parents are held to the same standards of courtesy, fairness, respect, and responsibility thrall we require of the players. We must lead by example if we are to expect the players to represent themselves, their families, their team, and the league in ways in which we can all be proud.
CODE OF CONDUCT FOR PLAYERS AND PARENTS

*I will represent myself, my family, my team, and the league with dignity, honesty, and integrity always.
*I will play with pride and intensity no matter the circumstances of the game and will always encourage my teammates to do the same.
*I will support my teammates and always maintain a positive attitude.
*I will be courteous and respectful to my teammates, coaches, and game officials.
*I will not argue with game officials for any reason.
*I will refrain from outbursts of anger, temper, and poor sportsmanship.
*I will never direct negative or critical cheers or comments toward anyone.
*I will not use vulgar or offensive language or gestures, regardless of the circumstances.
*I will not become hostile, nor will I resort to violence towards anyone.
*I will not use physical or verbal intimidation towards anyone.
*I will be properly and appropriately attired for participation on the field.
*I will be responsible for my own equipment as well as team equipment.
*I will do no harm to any facilities or fields, and I will not intentionally damage team equipment.
*I will not post inappropriate or negative comments on ANY social media outlet about my teammates, coaches, or opponents.
*No alcohol, drugs, or weapons of any kind.
*No significant others permitted in dugouts.

THE CONSEQUENCES

*All players, coaches, and parents are expected to always behave in a sporting manner. *Any player, coach, or parent/fans whose conduct is unbecoming or abusive will at a minimum be warned and may be removed from the playing field or stands at the discretion of the Tournament Officials or the Umpires.
*Foul or abusive language will not be tolerated under any circumstances. This includes a team forfeiting or being removed from the tournament if necessary.
*Failure to abide by the code of conduct may subject a player, coach, parent/guardian, or guest to one or more of the following disciplinary actions by the officers and directors of the league. Depending on severity and impact of the
occurrence, additional disciplinary action outlined below may be taken.
* An immediate warning from the facility by the Board Member on Duty
* A one game suspension during which you may not participate or be present as a player, coach, or spectator.
* A suspension for the remainder of the season in which you may not participate, or be present, as a player, coach, or spectator.
* A permanent suspension from any future participation or involvement in the league softball program.

All disciplinary actions and decisions will be made by the officers and directors of the league. There will be NO refunds of registration fees, or any other fees or portions thereof, when disciplinary action is taken. All decisions are final.
