Naples Elite Classic 2 May 23rd-25th

Consent: I, the parent or guardian of the above Travel team, does hereby give my approval to his/her participation in the Naples Spring Sunset Showdown. In case of injury to my child, I do hereby release, absolve, indemnify, and hold harmless the Naples Spring Sunset Showdown and all persons engaged in any manner in the basketball program from any and all claims and from any actions or negligence of the NYBL , its members or agents. I understand that I am responsible for accident insurance coverage for my child. I agree to allow my child's team photo, which may include my child, to be posted on the NYBL Elite and WNYBL website during the season.
I also agree to allow my child's first name, last name and division to be posted on the website. The names and photos will be removed after the season ends. In addition, by consenting, I agree to abide by the USSSA Code of Conduct (available on our website) Failure to abide by the Code of Conduct could result in participation being revoked.
